When using Boolean operators make sure you ALWAYS check for doubled points.
Some operators leave 2 points where there was one, some don't. Subtract is
one of the ones that does. I always run "Merge Points" whenever I use
boolean operators just to be sure.
The other possible problem I can think of is to merge the face polys of the
plaque, and to make sure all face points are really on the same plane. This
could also give you a problem such as you describe.
